Zorg connects values your privacy and with this privacy statement we would like to inform you how we handle your personal data. Protection of personal data is important to us and we ensure that data is always treated confidentially. We do this in accordance with the relevant privacy legislation.

Because you use Care Connects' services, you have provided us with certain personal data. This includes the following data that we process.

  • Indication (for the purpose of providing care).

  • Performance and assessment data

• Referrers

Care Connects processes your personal data for the following purposes:
Care Connects processes only those personal data we deem necessary to provide care and to select and manage our (new) employees.

1. To provide appropriate, tailored care.
2. To contact you by email or phone.
3. To inform you of changes regarding our services and products.
4. To provide services to you and deliver goods to you.
5. For sending our newsletter.
6. In connection with our legal obligations with respect to data processing with respect to, for example, tax authorities and the UWV.
7. To handle claims and payments.
8. Recruiting and appointing employees.

 

Retention periods:
Care Connects does not store your personal data longer than strictly necessary. Or no longer than the applicable legal retention period.

 

Secrecy
Care Connects handles personal data with care and always treats it confidentially. Personal data is only processed and used for the purpose for which it is needed and not given to third parties unnecessarily. Your personal data is handled by persons with a duty of confidentiality. Only employees who must have access to personal data in order to function may consult it and process it if necessary.

 

Sharing data with third parties
In case of cooperation with external parties (third parties) and involving data processing of personal data, Zorg connects a processing agreement. A processing agreement establishes the responsibilities in the processing of personal data between parties.

 

View, modify or delete
You have the right to view, modify or delete your personal data. You also have the right to data portability.

 

Objecting
You have the right to withdraw any consent you may have to data processing. You may also object to the processing of your personal data by Zorg verbindt. Care Connects ensures that your data is protected in the best possible way. For example, we take appropriate measures to prevent loss, misuse, unauthorized access, inappropriate disclosure and unauthorized modification.
